The following summarizes the format of the application:

1. Membership Grade Selection: Indicate your membership grade being applied for.
2. Name/Address: This should include phone/fax/email contacts.
3. Signature.
4. Education Background: Include copies of certificates and all relevant background.
5. Professional Affiliations.
6. Current Organization Tree.
7. Professional Work Experience.
8. Attestation by Proposers: Proposers must be two members of the association in good standing.
